Short Required Information Security Survey - and Chance to Win a NetBook!
UofL must comply with numerous information security related laws and regulations. An Information Security Risk Assessment is one of the expectations of many of these laws and regulations.
If you are responsible for:
- business administration of a school, division, department, unit, program or other administrative organization,
- technical or information systems support of a school, division, department, unit, program or other administrative organization, or
- academic or research activity.
Please go to https://sharepointsecure.louisville.edu/sites/infosec/initialra/default.aspx and log-in and select "Respond to Survey" to complete the survey. If you have questions about the survey or difficulties logging in, please see the FAQs below and, if required, contact Matt Witten at firstname.lastname@example.org or 852-0567.
The survey was designed and tested to be short (5 to 15 minutes, depending on the complexity of your environment) yet cover the university's legal and regulatory obligations so an effective risk assessment can be prepared.
As an incentive for timely response, all surveys completed on or before June 18, 2010 will be entered into a drawing for a free NetBook computer!

The survey is intended to assist the University in maintaining a high level of compliance with Information Security requirements. The results will be used to identify areas that may benefit from free information security consultation which will help to validate that good practices are in place or provide guidance on how to achieve good practices.
Areas that do not complete the survey will still be in the risk assessment pool and may be scored as higher risk due to a survey not being completed.
I can not log in, what should I do?
Easy, on the log-in box, type:
ad\userID (where userID is your GroupWise ID)
instead of simply your user ID. The "ad\" tells the system where to look for your log-in. This is needed if you do not have active directory installed on your computer and has worked for all users so far who could not log-in.
Where do I actually see the survey questions?
After logging in, there is a bar towards (but not at) the top of the screen that has "Respond to this Survey" on it. Please click on "Respond to this Survey" to go into the survey.
- I logged out or left the survey and need to get back into the same survey, what do I do?
- Go back to the survey at:
- Click on the InfoSec Initial Risk Assessment on the left side of the screen under 'Surveys'
- Click on "Show all responses"
Click on your response and then "Edit Responses"
This will get you back into your survey without creating an entirely new survey.
